Welcome to IHCCA

 Our goal is to provide information to all post-acute healthcare providers, caregivers and patients to fulfill our mission and achieve our commitments to Idaho individuals.

We are committed to:

• Allowing individuals to live and age in their residence of choice
• Provide safer transitions of care and education across the continuum of care, while reducing avoidable hospitalizations
• Represent the interests of the acutely ill, chronically ill, disabled, and dying Idahoan
